Armed Forces Benefit Association Insurance Program
Life Insurance Plans – the downloadable forms below can be filled in on-line using the Acrobat Reader. On some of the forms, only the required fields are fillable, non-fillable fields, with the exception of the signature line, are not required. Once the form is filled in, it must be printed and signed to be submitted. Filled forms cannot be saved.
$10,000 Plan for new Guard members: This plan is free for the first year to new members of the Guard. You should have received this information from your recruiter. If not, click on the link to get the application form. This plan covers the Guard member only.
Additional Life Plan: This plan is available to Guard members after the first year of service. Up to $50,000 in life insurance is available and family members can also be covered under this plan. Click the link to get the application form. If you have requirements for additional life insurance coverage contact our office directly at 317-247-3196 or via email at email@example.com.
Beneficiary Designation Form: Please fill out and submit this form along with the insurance plan application you select. Use this form whenever you need to update your beneficiary information.

American Benefits Plan
NGAI members are eligible for free notary services available at the NGAI offices during regular business hours. Hours are 0800-1600 Monday through Friday; closed on holidays.
- Must be a member of NGAI in good standing
- Must be able to produce 2 pieces of identification
- Documents must be signed at time of notary service
- $2.00 per notary fee for non-members (fee is donated to NGAI Professional Development Fund)
